Transferring Patient

If you would like to transfer care to our office or you are seeking a second opinion, your recent x-rays may be helpful. Please have your previous office send your x-rays directly to YOU via e-mail or a physical copy with the date they were taken. X-rays taken within the last 6 months are the most applicable.

Contact our office for further instructions once you have obtained your previous x-rays.
